Day One : Consult (40 min)

Step 1: Call 608-276-7635 or Email us at [email protected] to schedule a “Day One” appointment with us.

Step 2: We will email you paperwork to fill out before your appointment. Fill out our new patient paperwork online or by hand and return it to us 24 hours before your first appointment.

Step 3: Visit our office! You’ll get a short office tour before heading back to meet with Dr. Puckette for your Day One appointment.

Step 4: If you decide to move to the next step in our New Patient process, you will schedule a “Day Two” Exam.

Day Two : Exam (80 min)

Step 1: Visit our office! Dr. Puckette will meet you in our waiting area and bring you to his office for your in-depth mechanical and neurological exam.

Step 2: Together, you and Dr. Puckette will decide which course of treatment is the right fit for you. Each of our care plans is 100% customized and unique to fit each person’s needs.

Step 3: Meet with our patient coordinator to schedule your future appointments. Generally, we like to see you twice a week for 6 weeks during your first care plan.

Step 4: Go over your financial options with our patient coordinator! Ask any questions & take our financial plan packet home for review!

Day Three : The Journey Begins! (20 min)

Step 1: Visit our office! This is your first “typical” visit with us! You’ll be sent to one of our treatment rooms, where Dr. Puckette will meet with you for your adjustment.

Step 2: If you are a patient who has a plan that includes therapies with our Chiropractic Assistant, you will head to our therapy bay and our C.A. will teach you all of your recommended exercises!

Step 3: You’ll visit the front desk and let us know which financial plan you’d like to choose and set up payment with our Patient Coordinator.

Step 4: You come back for your next appointment to continue your journey to health restoration

Other Important Care Plan Information:

~Every 12 visits, you will have a “Progress Exam” to re-evaluate where you are in your healing journey and decide if there need to be adjustments to your care plan.

~If you have labwork done through our office, we will get you scheduled 2-3 weeks after the labs have been submitted for a lab review with Dr. Puckette. These appointments range from 40-60 minutes.

~If you are a patient who is recommended to use our nutritional services, you will schedule a Nutrition Exam and 2 Nutrition Check-in’s during your first 12 visits.
